Privacy Policy
Effective October 2, 2026. This notice describes the public Xtension Care website, consultation inquiries, and privacy requests. It does not describe every activity of a participating home care agency.
Information you provide
The consultation form collects your name, agency name, work email, optional phone number, service interest, preferred support hours, and message. The privacy form collects your email, request type, optional name, and optional details. We receive this information directly from you. Please do not submit medical information, passwords, identity documents, or financial account details.
Why these forms collect information
Consultation details are used to understand the support requested and enable business follow-up. Privacy-request details are recorded to identify the request and allow it to be reviewed and answered. A privacy request is not a request for a sales consultation. A submission confirmation means the record was received; it does not mean a deletion, correction, or opt-out has already been completed.
Website delivery and service providers
Vercel hosts the public website. Consultation and privacy submissions are forwarded to Xtension Care’s existing ChatGPT Sites inquiry service and stored there using its Cloudflare database infrastructure. Those hosting, application, and storage services process information to deliver and store submissions. Website infrastructure also receives technical information such as IP address, requested URL, browser information, and request timing for delivery, diagnostics, and security.
Advertising and browser privacy signals
The current public-website application has no advertising pixels, cross-context advertising integrations, or optional analytics scripts. Its form-handling code sends submissions to the inquiry service described above, not to an advertising platform. The Privacy Choices page can detect Global Privacy Control and explains this website’s current configuration. Legacy Do Not Track signals do not change the site’s functionality. Essential hosting and security processing still occurs.
Sale or sharing choices
The statements above describe this website’s configured processing; they do not characterize every offline activity or agency’s separate practices. Use “Do Not Sell or Share My Personal Information” to submit a request about identifiable records or ask about other processing. Submitting a request records it for review and does not automatically change another company’s systems.
Retention
The current inquiry service does not automatically expire consultation or privacy-request records; they remain stored until an authorized operator removes them. You can request deletion through the Privacy Request form. Applicable legal, security, or recordkeeping needs may affect whether a record can be removed. Infrastructure logs and backups are also subject to the relevant provider’s retention controls.
Access and security
Form submissions are transmitted over HTTPS. The forms do not make submitted records public. Access to stored information depends on the inquiry service’s authorized administrative access. No website, transmission, or storage system can be guaranteed completely secure.
Your requests
Use the Privacy Request form to ask about access, correction, deletion, sale or sharing choices, or other privacy questions. No CRM account or agency name is required. Provide only enough detail to identify the request. Additional proportionate verification may be needed before access, correction, or deletion; do not send identity documents unless an appropriate process is explained. Applicable rights and exceptions depend on the law and the role in which the information is held.
Separate CRM and agency information
Agency and Caregiver CRM workspaces are separate services with account permissions and agency-managed operational records. The public website form does not access or change those records. For a request involving an agency’s records, identify the agency in your request so responsibility can be determined, without including sensitive record contents.
Children and policy changes
This website is intended for adult business contacts, not children. If you believe a child’s information was submitted, use the Privacy Request form to report it without including unnecessary sensitive details. Updates to this notice will appear on this page with a new effective date. Contact us through the Privacy Request form about this notice.
