California Privacy Notice

Effective October 2, 2026. This notice supplements the public-website Privacy Policy for California residents. Rights depend on applicable law and Xtension Care’s role in the relevant activity.

Public-website information

Information categories handled by the reviewed website include identifiers and contact information, business or professional details supplied in the agency field, selected service interests, message contents, and technical request information. Sources are you and your interactions with website infrastructure. Purposes are website delivery, inquiry handling, privacy-request handling, diagnostics, and security.

Recipients and retention

The website uses Vercel hosting and the existing ChatGPT Sites inquiry service with Cloudflare database infrastructure. These services handle website requests and form submissions. Inquiry records have no automatic expiration in the current service and remain until an authorized operator removes them. Provider logs and backups follow provider retention controls. See the Privacy Policy for the scope of these disclosures.

Your California rights

Where the California Consumer Privacy Act applies, rights can include knowing and accessing personal information, requesting deletion subject to exceptions, correcting inaccurate information, opting out of sale or sharing, limiting certain uses of sensitive information, and not being discriminated against for exercising those rights.

How to submit a request

Use the Privacy Request form without creating an account. Select the request type and provide an email for a response; an agency name is not required. An authorized agent may identify their role in the details field. Access, correction, and deletion can require proportionate verification or proof of authority. The form does not require identity documents for a sale/sharing opt-out request.

Sale, sharing, and browser signals

The current public-website application has no advertising pixels or cross-context advertising integrations. Its form code forwards records to the inquiry service, not an advertising platform. The Privacy Choices page recognizes Global Privacy Control where the browser provides it. For choices involving identifiable records or other activities, use the request form. No claim is made that a submission automatically changes a separate agency’s or provider’s processing.

Agency-managed information

This notice covers the public website. Operational records in an agency workspace may involve different categories, legal roles, obligations, and exceptions. Identify the relevant agency when asking about those records. Do not include medical details or sensitive documents in the public request form.

Questions

Use the Privacy Request form to ask which information or activities this notice covers. The Privacy Policy explains the website’s current data handling. This notice does not represent that every CCPA obligation applies to every interaction.
